There are cases where records have been accumulating in the SYS:Action form where the default clean up workflow did not take some situations into account. The OOB works fine but does not take into consideration all data.
Below is the OOB workflow thats cleans data in the SYS:Action form.

What does the 'Action' field show for the accumulated records? CALCULATE_SLMDATES and UPDATESLMREASON data in SYS:Action are supposed to be deleted on submit so they should never be left on the form, but there have been sporadic reports of this data not being deleted.
The reason why it stuck in prior cases was that sometimes the operation that these data work on runs into error and rolled back and most of the time the action was completed but the deletion fails, this happens intermittently and seems to be environmental at times.
The data stuck on that form should not have any bad effect, but it does not serve any further purpose and can be removed.
